How to Search the Transition Hub

Find resources and high school transition programs by selecting your state and any other factors important to you (season offered, whether there is a fee, if a program has languages options other than English, or the type of program model: face-to-face, hybrid, virtual). If you select the default "All..." your search will provide all programs for that category.

If you don't find a transition program in your area listed on the APH ConnectCenter Transition Hub, please contact your state VR agency. Also, and please visit us again. The Transition Hub is young but growing, and new programs are added daily.
